Fatty liver disease in India

Non-alcoholic fatty liver disease is fat accumulating in the liver of someone who drinks little or no alcohol. It produces almost no symptoms in its early stages and is most often discovered by accident, on an ultrasound ordered for something else. It is strongly linked to insulin resistance, and in its early stages it is considered modifiable.

Why it is so easy to miss

The liver has no pain fibres in the tissue itself, so fat accumulation causes no sensation. Liver enzymes on a blood test can sit within the normal range while fat is accumulating. Most people who have it do not know, and most who find out did not go looking.

What makes it worth taking seriously is not the fat itself but where the process leads if nothing changes — inflammation, then scarring, and a cardiovascular risk that rises alongside it.

What drives it

Fatty liver travels with the same metabolic pattern as prediabetes: insulin resistance, abdominal fat and a diet high in refined carbohydrate and fructose. Two contributors get less attention than they deserve:

It is not only a problem for heavier people

Fatty liver is regularly found in people whose BMI looks unremarkable, particularly in South Asian populations where fat is carried viscerally. Common questions

Can fatty liver be reversed?
In its early stages, before significant scarring, fat in the liver is considered modifiable and often reduces with sustained changes to diet, weight and activity. Once fibrosis is established the picture is different, which is why finding it early matters. Your doctor, using imaging and blood work, is the one who can tell you which stage applies.
Can you have fatty liver without being overweight?
Yes, and it is not unusual in Indian populations. Visceral fat distribution matters more than total body weight, so a normal BMI does not rule it out.
Does fatty liver cause symptoms?
Usually not in early stages. Some people report fatigue or a vague discomfort under the right ribs, but most have nothing at all, which is why it is typically found incidentally on a scan.
Is alcohol the cause?
Not in non-alcoholic fatty liver disease, which is defined by its occurrence in people who drink little or no alcohol. Alcohol causes a separate form of liver disease, and the two can coexist.
